Effective March 1, 2012, every prescription written in the State of Delaware by a licensed healthcare provider must be written on a tamper-resistant prescription pad or paper approved by the board. Rxpads.com is an approved vendor through the State of Delaware.
All vendors must ensure that only licensed healthcare providers and authorized institutions are permitted to purchase prescription forms using the State of Delaware Provider Verification System (PVS).
When ordering prescription pads, you must supply the Provider Verification System Code for each provider listed on the pad.
